I spent the last while building Vocert: you record on iPhone/Apple Watch,and it seals the audio with a hash chain + signed, timestamped receipt. It also record where, when, and which advice it uses for recording Edit one byte and it fails verification — and anyone can check a recording in a browser (the verification is open, no account): vocert.com/verify also transcribes and pulls out the key moments.
